Fall Program Series to Help Patients Living with a Chronic Illness
Palliative Care Services at Jefferson Regional Medical Center is partnering with the Jewish Healthcare Foundation’s (JHF) program titled “Closure.” Together they will present a fall program series to help educate those living with a chronic illness and their families.
The curriculum for “Life’s Final Chapter: Have It Your Way” was developed to empower participants to make educated decisions by providing helpful resources and information. Speakers are all healthcare professionals who are members of the Coalition for Quality at the End of Life, convened by JHF. For more information about JHF’s initiative, visit www.closure.org .
Palliative Care Services at Jefferson Regional is led by Uzma Khan, MD, a board certified Palliative Care physician. She is assisted by Palliative Care team members from the disciplines of psychology, nursing, occupational and recreational therapy and spiritual services. Together they work to enhance the patient’s experience of living with a chronic illness.
“The focus of Palliative Care is to provide quality of life to those seeking medical treatment for chronic and life-limiting illness. The Closure series is designed to enhance life and assist in decision making,” said Dr. Khan. “We have no choice on the chronic illness that we face; we do have a choice on how we live with it.”
